Job summary 

Kennet and Avon Medical Partnership is a well-organised, friendly and forward-thinking Wiltshire training practice, looking for an Advanced Nurse Practitioner to join our expanding team.

This autonomous practitioner will have responsibility for managing our delivery of Primary Care services to the 5 care homes and smaller LD residential homes in our practice area. You will work with a nominated GP for each home to ensure all patients receive appropriate care and medication and have the relevant care plans in place.

In addition, you will work within a team to respond to requests for home visits and provide clinical expertise to patients by assessing and treating their medical condition.

Main duties of the job 

Job Summary: 

  • To act as an autonomous practitioner working independently and in conjunction with other health care professionals to assess, diagnose and treat the conditions of all registered patients, initiating direct referrals as appropriate.
  • To be responsible for managing the care for the patients registered in residential, LD and nursing homes covered by the practice boundary.
  • Assess and respond to requests for home visiting with the support of the triage team.
  • To develop new and innovative concepts, models, methods and practices to deliver new and improved primary care services to meet the needs of the practice, including education and training to other staff members and patients.
  • In conjunction with the doctors, managing minor ailments and provide face-to-face assessment and treatment of patients in line with business need and patient demand.
  • Duties may also include all tasks normally undertaken by an experienced RGN and in addition any roles agreed between the nurse and the doctors as appropriate, having regard to current training.

We are a dispensing practice delivering care to around 17,500 patients in the market town of Marlborough, the village of Pewsey and the surrounding countryside. The practice is situated within the heart of the North Wessex Downs Area of Outstanding Natural Beauty; has a range of excellent local schools and is close to the M4 and mainline railway services to London and the rest of the country.

We’re a friendly supportive team of 7 Partners, 3 Salaried GPs, 1 Retained GP and a GP Registrar. The wider multidisciplinary team within the practice includes our Paramedics, Triage Nurses, First Contact Physiotherapist, Practice Nurses who also undertake long term conditions management for us, Social Prescribers and a Health and Wellbeing Coach.

Qualifications 

Essential:

  • RGN qualification
  • Current NMC registration
  • Membership of the RCN or equivalent body

Desirable:

  • Independent Nurse Prescribing qualification
  • Accredited training in chronic disease management
  • Accredited training in anticoagulation management
  • Cervical cytology qualification

Patient Navigator

Our team of Patient Navigators receive, assist and direct patients in accessing the appropriate service or healthcare professional in a courteous, efficient and effective way. They also provide general administrative assistance to the Practice team and project a positive and friendly image to patients and other visitors, either in person or via the telephone.

In addition, Patient Navigators take a leading role in educating patients about how to best use the services on offer to them by the practice as well as other providers where appropriate.

Main duties of the job 

We are currently recruiting for a Patient Navigator to join our friendly team to provide an effective and efficient service to all patients and visitors to the practice both in person and via the telephone. The main duties of the role include taking telephone calls, greeting and directing patients, consulting with all members of the practice team as well as processing and recording appointment requests.

Excellent communication skills and attention to detail are essential. Experience of working within General Practice and with SystmOne would be desirable.
