Transforming Care of Older People (TCOP) Nurse

Job summary
We have an active Transforming Care of Older People (TCOP) Team that aims to proactively identify, manage and support frail and complex elderly patients to improve their quality of care and to release GP capacity by supporting these patients away from GP appointments. The team comprises a healthcare assistant and a social navigator led by the TCOP nurse. We are looking for an enthusiastic and highly motivated experienced nurse to lead this Team.
Main duties of the job
Identify patients that would benefit from more intensive management of their long term conditions, assess them and provide individualised care. Work closely with local nursing and residential homes as well as assisted living providers to support patients in these settings, as well as those in their own home. Being an Independent Nurse Prescriber is desirable, but not essential.
You will work closely with all members of the practice team, community services, social care, voluntary sector organisations and wider multi-disciplinary teams. This is an exciting post and the successful applicant will continue to build on the successes of the TCOP team.

Patient Navigator/Receptionist

Job summary
Our team of Patient Navigators (formerly called Receptionists) receive, assist and direct patients in accessing the appropriate service or healthcare professional in a courteous, efficient and effective way. The role of the Receptionist in General Practice has changed significantly in recent years to become a signposting service to direct patients to the most suitable healthcare provider, which may be the pharmacist, physiotherapist or urgent eyecare service to name but a few. They also provide general administrative assistance to the Practice team and project a positive and friendly image to patients and other visitors, either in person or via the telephone.
In addition, Patient Navigators take a leading role in educating patients about how to best use the services on offer to them by the practice as well as other providers where appropriate.
Main duties of the job
We are currently recruiting for a Patient Navigator to work 30 hours in our friendly team to provide an effective and efficient service to all patients and visitors to the practice both in person and via the telephone. The main duties of the role include taking telephone calls, greeting and directing patients, consulting with all members of the practice team as well as processing and recording appointment requests.
Excellent communication skills and attention to detail are essential. Experience of working within General Practice and with SystmOne would be desirable.

General Practitioner – salaried

Job summary
Are you looking for the opportunity to join a well-organised, friendly and forward-thinking Wiltshire training practice?
Would you like the chance to develop an interest in any of our additional services? Delivering GP care to a local large co-educational boarding school or involvement with medical services for an inpatient eating disorder unit perhaps?
Kennet and Avon Medical Partnership is looking for a Salaried GP and we are happy to be flexible regarding the number of sessions and working patterns for the right candidate.
Main duties of the job
Keeping our workload manageable is important to us. Duty sessions are well supported by an excellent triage team of Nurse Practitioners and Paramedics. Visiting rates are low, with the majority of visits being undertaken by our Advanced Nurse Practitioner or Paramedics. Workload is protected with pathology results and post all dealt with on your days off. Our very capable non-clinical team have also undergone training to take on much of the administrative work.
We offer a competitive salary, reimbursement of indemnity fees, 6 weeks of annual leave plus 1 week of study leave (pro rata) and ongoing mentorship with one of our Partners.

- To provide and maintain a high quality of care for patients as well as providing assistance to the doctors and other members of the primary healthcare team

Clinical Pharmacist

Job summary
KAMP are seeking two Clinical Pharmacists to optimise medicine usage across the practice by undertaking clinical medication reviews with patients, providing telephone advice for patients with medication queries, educating and supporting practice prescribers and auditing and improving practice prescribing procedures.
We are looking for someone who is eager to share their expertise, drive a culture of continuous learning to constantly improve our prescribing and put our patients first. If this sounds like you, we’d love you to join our expanding practice team.
Main duties of the job
You will be the medicines experts of the practice, working with our GPs, prescribing paramedics and nurses as well as our Dispensary to ensure medicines optimisation within our practice. Our PCN has a pharmacist working across 4 practices who can provide support and guidance.
You will achieve this through:
- clinical medication reviews with target patient groups including those with chronic conditions, residents of care homes or recently discharged from hospital
- annual medication reviews of those on repeat medication
- medicines reconciliation from hospital discharge summaries
- provision of telephone support service for patients
- improving practice prescribing procedures
- educating and supporting our prescribing practitioners
- implementing local and national guidelines and formulary recommendations, ensuring compliance with CQC standards as well as MHRA alerts and guidance
