Job summary

We are looking to recruit a new motivated and enthusiastic Social Prescriber Link Worker (SPLW) to join our dynamic primary care network (PCN) and work as part of a multi-disciplinary team within the East Kennet area of rural Wiltshire.

Our mission is to tackle mental health by focusing on developing a strong mental health service within the East Kennet area. There is an opportunity for the successful candidate to play a key role in supporting patients within this cohort.

Our PCN is a collaborative group of GPs whose surgeries are focal points within their communities.

Main duties of the job

The role will suit self-motivated individual who is proactive and can collaborate with different services. Training and support will be provided as required.

  • Promoting social prescribing and its role in self-management.
  • Build relationships with key staff across the PCN and becoming part of the wider network team.
  • Developing strong links with all local agencies to encourage referrals.
  • Work with all local agencies to raise awareness of social prescribing and how it can reduce pressure on statutory services.
  • Improve health outcomes and enable a holistic approach.

Social Prescribing can help to strengthen community resilience and personal resilience, by helping individuals to identify and tackle barriers to good health, such as debt, poor housing, loneliness and lack of physical activity and by increasing peoples active involvement with their local communities. It works particularly well for people with long term conditions (including support for mental health), for people who are lonely or isolated or have complex social needs which affect their wellbeing.

This role will cover all age groups, to include Children, Adults, Families and Carers.

Job summary

We are looking for an enthusiastic and forward-thinking person to join our friendly practice team. The successful candidate will be responsible for managing all elements of the practice finances and facilities for a busy, two site general practice.

Main duties of the job

  • Preparation and maintenance of monthly accounts
  • Maintaining the financial integrity of the accounting general ledger
  • Budget preparation and monitoring against monthly reforecast and key performance indicators, undertaking financial risk management as required
  • Analysis of both financial and non-financial information leading to insights needed to improve the profitability and performance of the business
  • Maintenance, review and reforecast of cash flow, taking appropriate action to mitigate any problems
  • Monthly payroll collation and processing, including pensions and HMRC
  • VAT calculations and returns
  • Commercial lead on Practice contracts
  • Liaise with external Accountants in the production of annual Partnership accounts
  • To oversee the Practice’s Facilities Management including Health & Safety
  • To ensure the daily smooth running of the practice’s computer systems, making recommendations for improving the Practice’s IT systems and processes, and leading on training as required
  • With the Practice Manager, develop internal control policies and procedures

Person Specification

Qualifications: Essential

  • Excellent standard of education with excellent literacy and numeracy skills
  • CIMA, ACA  Accounting Qualification or QBE

Experience: Essential

  • Experience of maintaining financial reporting and information systems
  • Experience of data analysis and the production of reports
  • Experience of financial planning and forecasting
